Development and Evolution of a Best-Practice Hospital-Based Care Transitions Program

Southwest General Hospital (SGH) in San Antonio, Texas, is a 327-bed acute care facility owned by IASIS Healthcare. In 2011, SGH partnered with Healthways in a six-month pilot study of Healthways’ newly developed hospital readmission avoidance program. Published studies on similar programs have reported substantial reductions in readmissions, higher patient satisfaction, reduced lengths of stay and extended relationships with patients.

The pilot study objectives were to determine and validate the feasibility and efficacy of new and emerging patient interventions designed to reduce readmissions in self-pay patients.
